 3. When you receive your voucher, take or send it to your supplier

4. Send us your feedback online or fill in the card provided in your pack.

Note: The voucher can be exchanged only for:

a. Reusable nappies, wraps, liners, booster pads, nappy inserts or nappy grips

OR

b. A nappy laundry service trial period

 To apply for your pack you will need to complete a form and provide evidence of:

where you live, such as

 council tax

 water

 gas

 electricity

 or landline phone bill

your identity such as

 driver's licence

 passport

 birth certificate

 pregnancy or birth of your child - your MatB1 form or baby's birth certificate

Please provide copies, not originals, of these documents.

Voucher pack application Please Note:

To be eligible for a voucher You should be a resident of a participating borough.

You should be expecting a baby or have a baby not older than 18 months. As well as completing this form you need to provide us with evidence of: your identity: eg driver's licence, passport, birth certificate address, (in your name): eg council tax, water, gas or electricity, landline/mobile phone bill, bank statement pregnancy or birth: eg MatB1 form (both sides), NHS prescription charge card, baby's birth certificate
